As a new patient
Before deciding to register with our practice please check to see how our surgery works. You can do this by looking at the appointments page, asking at reception for a practice leaflet or asking reception any questions you may have. If you feel happy to register at this practice we will then discuss what process you need to take.
We are able to take on any patients new to the area providing you live within the catchment area we cover. The practice covers the town of Coalville and also the following villages:
- Ellistown (only part of village check with practice)
- Swannington (only part of village check with practice)
Please check you are in our catchment area as the catchment area checker is just a rough idea of our area.
If you hand a regsitration form in when you are out of our area your registration will be refused.
Registration forms are obtainable from the website and reception which need to be completed and returned to the practice with proof of identity.
You will need to complete the following forms:
Summary Care Records Consent Form - you will need to ask reception for one of these.
If you have a communication need of any of the following please inform the practice when you register:
Online access form (optional) - also obtained via reception.
If you would like to print these forms from the website and then bring to the surgery ith 2 forms of ID you can but please remember to ask for a summary care record consent form to complete your registration.
Please make sure you print and complete all forms or your registration will be refused.
Proof of Identity
It is Practice Policy that we will need to see two forms of identification for each new patient registering with this practice.
One will need to have a photograph of the patient and the other must have their current address.
Acceptable forms of ID are:
- Birth Certificate
- Driving Licence
- Marriage Certificate
- Local Authority Rent Card
- Paid Utility Bill
- Letter from Benefits Agency/Benefit Book/Signing on Card
For babies only please bring birth certificate and red book
- Library Card
- Video Rental Card
- Private Rent Book
As a temporary resident
We will need to see one form of ID. This ID can be any of the ones above or a bank card is acceptable for temporary residents only.
Attached to the registration form is a brief health questionnaire which also has to be completed.
On return of the completed registration form, health questionnaire and we are happy with the proof of identity we will then be able to process the application.
All new patients over the age of five years can have 20 minute appointment with the nurse for a New Patient Medical.
At present we have an "Open List" so we are also able to take on patients currently registered with another doctor within our area subject to them meeting the conditions as above.
